What to Bring to Your First Appointment:

  • A valid government-issued photo ID
  • Your insurance card(s)
  • A list of current medications (including supplements)
  • Any relevant medical records or prior test results
  • A list of your current symptoms or concerns
  • Your co-pay or payment method if applicable

What to Expect:

Your first visit will include a comprehensive health history review, a physical assessment, and a conversation with your provider about your health goals. If you are visiting for a CDL/DOT physical or as a military service member, please let our team know when booking so we can prepare appropriately. Plan for approximately 45–60 minutes for your first appointment.

Access Your Patient Portal

Our patient portal is powered by Athena Health, a leading healthcare platform. Through your Athena Health portal, you can access your health information securely and conveniently. Through the portal, you can:

  • View and manage your appointments
  • Access lab results and health records
  • Request prescription refills
  • Send secure messages to your care team
  • Review your visit summaries and care plans
  • Pay your bill online

Insurance & Billing

We accept a wide range of insurance plans to ensure that cost is not a barrier to quality care. We also offer flexible cash-pay options for uninsured patients and select services. Most insurance approvals are expected by May 2026. Plans are being added on a rolling basis. Please contact us or your insurer to confirm your coverage status before scheduling.
